Intake Assessment

Every client receives a comprehensive 60–90 minute intake covering:

  • Fall history and near-fall episodes over the past 12 months
  • Medication review (with attention to fall-implicated drug classes)
  • Vision screening (referrals made when indicated)
  • Vitamin D, hydration, footwear, and lifestyle factors
  • Cognitive screen and dual-task readiness
  • Red-flag screen β€” vestibular, Parkinson's, peripheral neuropathy, and neurological disorders are referred back to the appropriate specialist; our program is not a substitute for that care.

Standardized Physical Measures

  • Timed Up and Go (TUG) β€” dynamic mobility and fall-risk cutoff
  • 30-second Sit-to-Stand β€” lower-extremity strength and endurance
  • Single-Leg Stance (open/closed eyes) β€” static balance and postural control
  • 4-Stage Balance Test β€” CDC STEADI–aligned
  • Gait analysis β€” cadence, step length, symmetry, turning
  • Selected Berg Balance Scale items as clinically indicated

Home Environment Assessment

An in-home visit identifies environmental risk factors and produces a written, prioritized list of modifications β€” from lighting and rug removal to bathroom safety and stair safety.

Re-Assessment Cadence

Standardized measures are re-scored at mid-program (week 4) and graduation (week 8), with a written comparison to intake shared with the referring provider on consent.
